- A consular certification application form, as per form LS/HPH-2012/TK, or a printed version from the website lanhsuvietnam.gov.vn.
- The original identification documents: Identity card, passport, or valid substitute documents (if submitting directly). If submitting by post, a photocopy of the identification document is required.
- Documents or materials requested for consular certification: Provide the original or a notarized copy.
- One photocopy of the document/material requested for consular certification.
- One envelope with the recipient's address clearly written (if the documents are sent by post and the result is requested to be returned by post).
When You Need Consular Certification for Foreign Documents to Be Used in Vietnam
